Observation

During the complaint assessment it was reported by the previous Assistant Director that an incident report was not provided to the child's parent for the allergic reaction. It was also reported by the Center Director that an incident report was not provided to the parent for the child's allergic reaction or the black eye that the child had when dropped off the following week.
